cancellation & No Show policy

We understand that sometimes it may be necessary to cancel or reschedule an appointment. We kindly ask that you give us at least 24 hours notice if you need to cancel or reschedule your appointment.
 
If you fail to provide at least a 24 hour notice or do not show up for your scheduled appointment, you will be charged a cancellation fee of 50% of the scheduled service(s). This fee is necessary to compensate our staff for their time and to cover the cost of any supplies that have been allocated for your appointment.
 
If you have purchased a package of services, please note that missed appointments, or appointments cancelled with less than 24 hours’ notice, will be deducted from your package.
 
By scheduling an appointment with us, you agree to our cancellation policy. We reserve the right to modify our cancellation policy at any time without prior notice.

late policy

If you are running late for your appointment, please call us as soon as possible. If you arrive later than scheduled, we will make every effort to accommodate your full appointment but this is not always possible. Service time may be adjusted to avoid delays for other guests.

forms & consent

All required forms and consents will need to be signed prior to receiving any services. This will be done in office or online via our Boulevard software system. Arriving late will simply limit the time for your appointment. Your appointment will end on time so that the next guest is not delayed. Patients arriving more than 15 minutes late for their appointment will need to reschedule.

Payment

We accept all major Credit Cards American Express, Visa, Mastercard, Discover, Care Credit.
 
We do not accept personal checks, money orders, direct bank deposits.
